What Is an Illinois DUI Evaluation?
An Illinois DUI evaluation is an alcohol and drug evaluation completed following a DUI arrest or for certain court, probation, or driver’s license requirements.
The evaluation considers the DUI circumstances, chemical-test information, driving and legal history, alcohol and drug-use history, previous services, objective assessment results, supporting records, and the clinical interview.
This information is used to determine the applicable Illinois DUI risk classification and recommendation. A DUI evaluation is not a pass-or-fail test. The result is based on the complete information available and the individual circumstances of the case.
When Is an Evaluation Needed?
An Illinois DUI evaluation may be needed for:
- Court proceedings or sentencing
- Court supervision or probation requirements
- Attorney-requested case preparation
- Illinois Secretary of State hearings
- Driver’s license relief or reinstatement matters
- Certain out-of-state DUI or OWI matters

What Should I Provide?
Be prepared to provide available documents such as:
- Valid photo identification
- DUI ticket or charging documents
- Law-enforcement and chemical-test records
- Court, probation, or attorney instructions
- Driving record or abstract when required
- Information concerning previous DUI offenses
- Prior evaluation, education, or treatment records
- Secretary of State correspondence when applicable
What Happens During the Evaluation?
- Contact the office. Explain who requested the evaluation and receive scheduling and document instructions.
- Provide the requested records. Complete records help the evaluator accurately review the DUI, driving history, and previous services.
- Complete the interview and assessment. The appointment includes a substance-use history, discussion of the DUI circumstances, and required objective assessment procedures.
- Receive the classification and recommendation. The evaluator reviews the available information and completes the applicable Illinois documentation.
Evaluation records are released to authorized recipients in accordance with applicable requirements, agency policy, and valid client authorization.
Illinois DUI Risk Classifications
Illinois DUI evaluations assign one of four risk classifications:
- Minimal Risk
- Moderate Risk
- Significant Risk
- High Risk
The classification is based on the complete evaluation—not one isolated answer—and helps determine the applicable education, intervention, or treatment recommendation.

If the evaluation recommends services that our agency does not currently provide, those services may be completed through another appropriately licensed provider.
