A Uniform Report Update provides current or additional information related to an existing Illinois DUI Alcohol and Drug Evaluation Uniform Report. An update may be needed after recommended services have been completed or when additional documentation is requested by a court, probation office, attorney, or the Illinois Secretary of State.
The update does not automatically replace the original DUI evaluation. The appropriate service depends on the existing evaluation, the information that must be updated, the requesting party, and applicable Illinois requirements.
